Near misses in logistics
Warehouse, loading bay, vehicle fleet: wherever people and vehicles share the same route, something almost goes wrong nearly every day. Almost none of it gets reported.
Who it is for
Warehouse management, shift leads, drivers, forklift operators and the safety officer. Anyone who can scan a QR code can report, including temporary staff and contractors without an account of their own.
How it works today
A forklift brakes hard because someone steps out from behind the racking. Both carry on, nothing happened. The form is in the office, the shift is ending, and by the time anyone writes it up it is forgotten. What remains are the cases where something did happen.

QR codes hang at the loading gate, at the end of the racking, in the driver’s cab. Scan, talk for thirty seconds, done, without signing in, without a password.
The AI sorts it out
“The forklift came round the corner and suddenly I was standing in front of it” becomes area, activity, cause and risk potential. Two follow-up questions at most, with tags to tap, operable with gloves on.
Single cases become a pattern
From the third similar report the analysis recognises a pattern and names the focus: on-site traffic, for example, mostly at the loading gate, mostly under time pressure.
